As we prepare to close our foundation, we remain committed to serving eligible families through the conclusion of our program. Please review all requirements carefully before submitting.
Eligibility Requirements
✓ New Jersey resident
✓ Child (age 0–19) currently in active treatment for pediatric cancer
Document Requirements
✓ Signature page completed and uploaded with the application
✓ Invoice shows customer's full name and address
✓ Invoice dated within 60 days and includes account number, balance due, payment address, and payee name
Approval & Payment
Payments are issued once per month. Requests received during a given month are generally processed in the first week of the following month. Funeral expense requests are given priority and processed as promptly as possible.
Please note: Incomplete applications or unclear documentation may prevent processing before the program concludes. Review all materials carefully before submitting.
Important Deadline
September 30, 2026 is the final day to submit a request.
Applications received after this date will not be accepted.
